+formulaire [ID]: Creates a form that members can complete by pressing a button, answers are then sent to a log channel
The panel sets the title, the questions (5 at most), the button's channel and message, its text, emoji and colour, and the channel where the answers arrive. With the ID of its button's message, it edits an existing form.
